The Cook Country Health Foundation (CCHF, a non-for profit entity) (the “Foundation”, “we” or “our”) is committed to respecting the privacy of visitors to www.cchealthfoundation.org. This privacy policy discloses the privacy practices regarding information we collect from visitors and the use of this information. By using the Foundation’s Web site, www.cchealthfoundation.org, you confirm that you have read, understood and agreed to both this Privacy Policy and the Terms of Use. Your use of the Site and any disputes concerning privacy are subject to this Privacy Policy and the Terms of Use. DO NOT USE THE SITE OR PROVIDE INFORMATION TO THE FOUNDATION IF YOU DO NOT AGREE WITH THIS PRIVACY POLICY AND ALL OF THE TERMS OF USE.

Information Gathering and Use
We collect two categories of information from visitors to the Site: personal information and non-personal information. Personal information consists of information that can identify you as an individual. Non-personal information is information that does not reveal your personal identity. This type of information includes aggregate information (i.e., information presented in summary or statistical form that does not readily reveal an individual’s identity).

Only in a few instances does the Site require you to submit personal information in order to access its pages. In those instances, the purpose is to provide you with the most personalized Web experience possible. By knowing how to contact you, we can deliver relevant information in the areas you indicated.

The Site may also require you to provide personal information in order to submit content or materials to the Site or to download content or materials from the Site. In those instances, the purpose is to allow us to keep track of the identity of the user who agreed to the terms and conditions associated with such submission or download.

In all cases, it is your choice and at your discretion whether to provide personal information. Personal information you provide will not be forwarded, traded, or sold to outside organizations. We, however, reserve the right to share information we collect with our affiliated entities. If you provide contact information, you may be invited to register and “opt in” to future communications by email. If you choose to register, we reserve the right to contact you at any time to forward information, answer questions you submit, or notify you of any changes in our registration policy. If you do not wish to receive correspondences from the Foundation, you will have the option to unsubscribe from the mailing list.

You acknowledge and agree that we may disclose information you provide if the law requires, or if we, in our sole discretion, believe that disclosure is reasonable for any of the following reasons: (1) to comply with requests or orders from law enforcement or any legal process (whether or not such disclosure is required by applicable law); (2) to protect or defend the Foundation’s or a third party’s rights or property; or (3) to protect someone’s health or safety (e.g., when there is a threat of harm or violence against you or any other individual).

“Cookie” technology is used by the Foundation to improve the Site and to deliver a better and more personalized experience. Cookies are small files issued by the Site and stored on your computer’s hard drive to provide us with information about your general use of the Internet. Personal information (such as a user’s identity) is NOT stored in cookie format and cannot be accessed by “peeking” at a user’s cookies. You may refuse to accept cookies by activating the appropriate setting on your Web browser. However, if you select this setting you may be unable to access certain parts of the Site’s services and features.

The Foundation uses Web software to gather statistics about the Site’s usage and to determine trends. These statistics provide information concerning the number of visitors to the Site and the most-visited pages. To gather these statistics, we collect at least the following information from you: your computer’s IP address, the Internet address of the site from which you came and the pages you visit on the Site. No personal information is collected during the tracking process.

Links to Other Sites
To aid in the Foundation’s cause and to raise money, we provide links on the Site to useful third-party resources. Please note the linked sites have different privacy practices. When leaving the Site, we recommend that you check the privacy policy of the site you are visiting before sharing information. We have no control over and are not liable for any information you provide to third parties.

Protecting Children’s Privacy
We do not intend to collect any information from children under the age of 13. In compliance with the Children’s Online Privacy Protection Act (COPPA), we will promptly purge any personal information in our database from users that we believe are under the age of 13.

Your California Privacy Rights
California Civil Code Section 1798.83 (also known as the “Shine The Light” law) permits users who are California residents to request and obtain from us once a year, free of charge, information about the personal information (if any) we disclosed to third parties for direct marketing purposes in the preceding calendar year. If applicable, this information would include a list of the categories of personal information that was shared and the names and addresses of all third parties with which we shared information in the immediately preceding calendar year. Information Security and Access
In an effort to protect your personal information, we store the personal information we collect in a secure area, and we will exercise reasonable care in providing a secure transfer of information between your computer and our servers. In spite of these precautions, we do not guarantee the security of your personal information or other information in any form and are not liable for the purposeful or accidental disclosure of this information.

Amendments
If we decide to change our Privacy Policy, we will post changes to this Privacy Policy page and any other places we deem appropriate. We reserve the right to modify this Privacy Policy at any time, so please review it frequently.

Your continued use of any portion of the Site, following a posting of changes to this Privacy Policy or an updated version of this Privacy Policy, will constitute your acceptance of the changes or updates.
