This year, Cook County Health Foundation presented our Partner of the Year award to the team at Belmont-Cragin Health Center for their exceptional work with recent arrivals. This award is a recognition bestowed upon a partner of the foundation’s who has demonstrated outstanding performance, exceptional leadership, and a devotion to our shared values. This award celebrates the partner’s ability to drive innovation, foster collaboration and deliver exceptional results. It serves as a testament to their decision, expertise, and unwavering commitment to humanity, equity and community.

THE TEAM AT BELMONT CRAGIN
Belmont Cragin Health Center, which opened its brand new facility in 2021, is one of a dozen community health centers operated by Cook County Health across Chicago and Suburban Cook County. Within a day of receiving a call from the City of Chicago and State of Illinois, Cook County Health established a clinic inside the Belmont Cragin Health Center to exclusively serve the needs of new arrivals. The clinic has served more than 14,000 men, women and children since August, 2022.
